The start of a new year often comes with quiet promises to ourselves: this year will be different. For millions of people who smoke, that promise includes a desire to quit – not just for longevity, but for better breathing, better oral health, and a better quality of life.

Quitting smoking is one of the most powerful health decisions a person can make. Yet we also know the reality: quitting is hard, and traditional “quit-or-fail” approaches don’t work for everyone.

If you struggle to quit, reduce the harm and live to fight another day.

Quitting Doesn’t Have to Be “All or Nothing”

Decades of research now clearly show that the greatest harm from smoking comes from burning tobacco, not from nicotine itself. Combustion releases thousands of toxic chemicals that damage the lungs, heart, gums, and teeth.

THR is a pragmatic, compassionate approach that recognises this reality. Instead of demanding immediate abstinence, which many people struggle to achieve, THR encourages smokers who cannot quit straight away to switch to far less harmful, non-combustible alternatives as a step toward quitting altogether.

What Does the Evidence Say?

A major Cochrane Review published in 2025, widely regarded as the gold standard for healthcare evidence, found no evidence of serious health harms associated with switching from cigarettes to tobacco-free oral nicotine pouches. Notably, the review also confirmed their potential role in helping people stop smoking, while noting that continued research is ongoing.

Cochrane has also consistently found that nicotine e-cigarettes are among the most effective tools available to help adults quit smoking. This reinforces a clear public-health message: non-combustible nicotine products can play a valuable role in helping people move away from smoking altogether.

In parallel, the international expert report Cigarettes to Smiles highlights how switching away from smoking can deliver real improvements in oral health, including reduced gum inflammation, less staining, and lower exposure to cancer-causing substances. Countries like Sweden, where safer nicotine alternatives are widely available, now have some of the lowest smoking rates and smoking-related disease levels in Europe.

A Healthier Path Forward

To be clear: the best option is always to quit smoking and nicotine entirely. But for people who have tried and struggled, or who are not ready to quit overnight, switching away from cigarettes can dramatically reduce harm and create a realistic pathway toward becoming smoke-free.

This New Year, quitting smoking doesn’t have to mean perfection, it can mean progress. Every cigarette not smoked is a step toward better health, and every step counts.
